Why Thin, Healthy People Can Still Have High Cholesterol

You run three times a week. You cook most of your own meals. The scale has said roughly the same number for a decade. And then a routine blood panel comes back with an LDL that your doctor wants to talk about. It feels like a contradiction, because so much cholesterol messaging is really weight messaging in disguise. But LDL cholesterol and body weight are only loosely connected, and plenty of lean, active people carry numbers that surprise them.

Cholesterol isn't a weight problem, it's a clearance problem

Most of the cholesterol circulating in your blood isn't the cholesterol you ate. It's made by your liver, packaged into LDL particles, and either used by your body or pulled back out of the bloodstream by LDL receptors on liver cells. Weight gain can make that system less efficient, which is why cholesterol and weight often move together in population-level studies. But the receptor system itself is largely genetic, and it works the same way whether you're at a healthy weight or not. If your liver isn't clearing LDL particles efficiently, being lean does not fix that on its own.

When it's genetic: familial and polygenic hypercholesterolemia

The clearest example is familial hypercholesterolemia, or FH, an inherited condition in which the LDL receptor doesn't work properly from birth. According to the CDC, heterozygous FH affects roughly 1 in 250 people, which adds up to more than a million people in the United States and an estimated 25 million worldwide. Most of them don't know they have it. FH typically shows up as an LDL well above 190 mg/dL, often present since childhood, in someone who otherwise looks and lives like the picture of health.

  • It runs in families: a parent, sibling, or child with early heart disease or a similarly high LDL is a meaningful clue, even if everyone in the family is lean.
  • It's not about lifestyle: diet and exercise still help, but they usually aren't enough on their own to bring FH-range LDL into a normal range, which is why it's worth naming out loud to a doctor rather than assuming more salad will solve it.
  • Polygenic hypercholesterolemia is more common and milder: instead of one faulty gene, dozens of common genetic variants each nudge LDL up a little. The combined effect can still land someone in a concerning range despite a clean diet and a healthy weight.
  • It's underdiagnosed on purpose, in a sense: doctors typically look for FH using clues like a very high LDL paired with a family history of early heart disease, not body size. If nobody has ever asked about your relatives' heart health at a checkup, that's a gap worth raising yourself, especially if a first-degree relative had a heart attack or stroke before age 55.

Other reasons your numbers can climb without any weight change

Genetics gets most of the attention, but it isn't the only non-weight explanation. A few others show up often enough to be worth knowing about.

  • Age and hormones: LDL tends to rise gradually through adulthood in both sexes, independent of weight. In women, the shift around menopause is well documented: a study of nearly 3,000 women (the SardiNIA study, published in Maturitas) found postmenopausal women had measurably higher total cholesterol, LDL, and triglycerides than premenopausal women at similar ages, tied to declining estrogen and changes in thyroid hormone levels that both affect how quickly the liver clears LDL from the blood.
  • Thyroid function: even mildly underactive thyroid function, which can be easy to miss, slows LDL clearance and is a standard thing doctors check for when cholesterol is unexpectedly high.
  • Very low-carb or ketogenic eating: some lean people on strict low-carb diets see dramatic LDL increases, an effect that's been documented in case reports and is now an active area of research. If you've recently changed how you eat, that timing is worth mentioning to your doctor.
  • Normal-weight visceral fat: body fat percentage and where fat is stored matter as much as the number on the scale. Research on normal-weight obesity, including a study published in the journal Nutrients on young adults with normal BMI but elevated body fat, found this group had higher triglycerides and LDL than their normal-weight, lower-body-fat peers, even though both groups looked identical on a scale.
  • Smoking: it raises LDL and lowers HDL regardless of body composition, one of the more reversible items on this list.

What this means for you

If a high LDL result doesn't match how you look or feel, the instinct to think that can't be right is understandable, but it's worth treating the number as real information rather than a mistake. It also helps to remember that this mismatch is common enough to be unremarkable to a doctor who sees it regularly. You are not the first lean, active person to sit across from a clinician confused about a lipid panel, and you won't be the last. A few concrete next steps:

  • Get the fuller picture, not just LDL: ask whether ApoB or Lp(a) testing makes sense. ApoB counts the actual number of cholesterol-carrying particles, which can tell a different story than LDL alone, especially in lean people. Lp(a) is largely genetic and isn't affected by diet or weight at all.
  • Map your family history: early heart attacks, strokes, or high cholesterol since childhood in close relatives are worth mentioning even if no one in the family was ever overweight.

What you can control is what happens next: getting the right tests, understanding your family history, and tracking whether the changes you do make are actually moving your numbers.
